Health & Medication.
Anything the school nurse needs from a parent or provider.
- Authorization for Administration of Medication at SchoolRequired before a school nurse can give your child any medication — prescription or over-the-counter. Signed by both parent and prescriber.
- Allergy Care Plan and OrdersFor students with diagnosed food or environmental allergies. Provider-signed.
- Asthma Care Plan and OrdersFor students with diagnosed asthma. Provider-signed.
- Request for Special Dietary AccommodationsUSDA-required form for diet-related accommodations in school meals. Provider-signed. See OSPI for the current statewide form.
- Certificate of Exemption (Immunization)WA Department of Health form. Required if your child is exempt from one or more required immunizations.
Athletics.
Most athletic forms now run through FinalForms — these are the legacy paper backups and statutory acknowledgments.

Athletic registration happens in FinalForms, the district’s online system for sports paperwork. Parents create an account, sign the required forms electronically, and upload the sports physical (good for 24 months). Register there before the first practice — the forms below are only the statutory notices and paper backups.
