FinalForms registration
All Montesano athletes register through FinalForms — a secure online platform that consolidates the physical, code of conduct, concussion info, cardiac arrest awareness, and family contacts in one place. Once you're set up, registering a second sport in the same year takes about 5 minutes.
Required forms
- Physical exam — valid for 24 months from date of exam, signed by a licensed health care provider.
- Athletic Code of Conduct — student and parent acknowledgment.
- Concussion & Sudden Cardiac Arrest acknowledgment — required by Washington law (Zackery Lystedt Law).
- Emergency contact & insurance information.
- Free & Reduced sports fee waiver — submit through the District Office if applicable.
Eligibility
WIAA eligibility requires students to be passing at least 5 classes (or 80% of the class load) at the most recent grading period. The athletic department runs eligibility checks each grading period and notifies coaches and families.
Concussion protocol
Athletes suspected of a concussion are removed from play immediately and may not return until cleared in writing by a licensed health care provider, per RCW 28A.600.190.
